The register contains the following particulars relating to a medical practitioner:

• full name;

• date of birth and national registration number;

• personal address;

• business address;

• the date of registration;

• description and date of the qualifications which he possesses on the date of registration; and

• such other particulars as the Council may determine.

A medical practitioner registered under this Act shall inform the Registrar of any change in that person’s address or in any other particulars relating to him.
